Dear Veterans,

Greetings!

I am the Director of the Arizona Department of Veterans' Services.

Do you want to improve transportation for military Veterans? Access to transportation is key to the independence, health and wellness of the over 500,000 military Veterans and their families who call

Arizona home. Particularly in rural areas, unreliable transportation is associated with more rescheduled and missed healthcare appointments as well as the inability to shop for groceries, seek employment and other opportunities that allow Veterans to thrive.

The Arizona Veteran Transportation Matters program is a community-action initiative between the Arizona Department of Veterans’ Services (ADVS) and the University of Arizona. The goal is to hear from Veterans in Arizona about their transportation needs. This information will be used to generate potential solutions to improve transportation. Your voice matters.

The survey is 10 minutes and can be completed on a smartphone, tablet, or computer. A caregiver can help a veteran take the survey.

To complete the survey, go to https://azvetsurvey.online/zec or scan the QR code:

Email coe-va@email.arizona.edu with questions about the survey.

Our goal is 10,000 surveys. We look forward to your participation! Thank you for your time.

If you or any service member, veteran or family member you know needs help connecting to support or resources, please reach out to our Be Connected program at: www.BeConnectedAZ.org or 1-866-4AZ-VETS (429-8387).

Sincerely,

Wanda A. Wright,

Director
